How to read multivitamin doses nutrient by nutrient

FDA Daily Values help compare labels, but they are not personalised RDAs. NIH notes that RDAs and AIs vary by age, sex and nutrient, while FDA generally uses one DV for adults and children age 4+. [1] [2]

A multivitamin therefore has no valid total-active milligram dose. Calcium, iron, vitamin D, folate and B12 each need their own amount, unit, form and reference context. Prenatal and older-adult formulas should not be scored using the same expectations as a general adult product.

What multivitamin evidence does — and does not — show

Multivitamins can increase nutrient adequacy, but NIH also notes that some users exceed upper intake levels when supplements are combined with diet and other products. [1]

Long-term randomized evidence has not shown cardiovascular prevention: Physicians' Health Study II found no reduction in major cardiovascular events. [5] The same trial found a modest reduction in total cancer incidence, but USPSTF still concludes that evidence is insufficient to recommend multivitamins for prevention of cardiovascular disease or cancer in community-dwelling nonpregnant adults. [4] [6]

COSMOS cognitive studies provide more encouraging results in older adults using a specific older-adult multivitamin, but those findings should not be generalized to every formula. [7] [8]

Filling nutrient gaps

Multivitamins can help people reach recommended intakes when diet is inadequate, but formula and diet determine whether they correct gaps or create excess. [1]

Heart disease, cancer and longevity

Evidence is insufficient for recommending a multivitamin to prevent CVD or cancer, and a 2024 cohort analysis found no mortality benefit. [4] [9]

Cognition in older adults

COSMOS-Web and COSMOS-Clinic found modest memory/global-cognition benefits with a specific commercial multivitamin/mineral in older adults. [7] [8] This does not establish a class-wide cognitive effect.

When to take a multivitamin

No single timing rule applies to every multivitamin. Follow the product directions and any medication-specific instructions. Food can improve tolerance for some products and is commonly recommended on commercial labels, but timing does not solve nutrient overlap or interaction issues.

How to compare multivitamin testing evidence

A strong multivitamin quality programme should verify many declared nutrients, relevant chemical forms, contaminants, batch identity and finished-product performance. Testing only one nutrient or only heavy metals does not verify the Supplement Facts panel.
