How to read multivitamin doses nutrient by nutrient

UK multivitamin labels use nutrient-specific NRVs, not a single multivitamin target. [2] A product may provide 100{3683f0b818e4e7eac3a25ad8212965194ab09621a23bd0189e97e17dfedff11f} NRV for zinc while only 16{3683f0b818e4e7eac3a25ad8212965194ab09621a23bd0189e97e17dfedff11f} NRV for magnesium; adding those percentages together would be meaningless.

NRV is also not always the same as current UK intake guidance. For example, the vitamin D label NRV is 5 mcg, while NHS guidance says adults need 10 mcg/day and should consider a 10 mcg supplement during autumn and winter. [4] Iron requirements differ by sex and age, and pregnancy has separate folic-acid/vitamin-A guidance. [7] [8]

For each product, calculate every nutrient separately from the labelled daily serving. Never create a total-active milligram figure.

What multivitamin evidence does — and does not — show

Multivitamins can help increase micronutrient intake when diet does not supply enough, but they can also push some nutrients too high when combined with fortified foods or separate supplements. [11]

Long-term randomized evidence does not show a broad cardiovascular benefit: the Physicians' Health Study II found no reduction in major cardiovascular events after a median 11.2 years. [12] The same trial found a small reduction in total cancer incidence, but not a broad reduction in site-specific cancers or cancer mortality. [13]

Newer COSMOS studies suggest modest cognitive benefit in older adults using a specific older-adult multivitamin, but this does not establish that every formula improves cognition. [14]

Filling nutrient gaps

A multivitamin can raise intakes of nutrients that are low in the diet, but adequacy and excess must be assessed nutrient by nutrient. [11]

Heart and cancer prevention

Current trial evidence does not justify selling a general multivitamin as cardiovascular prevention. Cancer evidence includes a small signal in one major trial but is not a universal prevention result. [12] [13]

Healthy ageing and cognition

COSMOS cognitive studies in older adults found modest benefits with a specific commercial multivitamin/mineral. [14] The evidence is audience- and product-specific.

When to take a multivitamin

There is no one evidence-based time of day for every multivitamin. Follow the product directions. Taking a multi with food can improve tolerance and may help absorption of fat-soluble vitamins, but medicine-specific timing instructions take priority.

If a multivitamin contains iron, calcium, magnesium or vitamin K, check medicines and other supplements rather than assuming all ingredients can be taken together.
