Vitamin K2 labels are easiest to compare by form first, dose second. Keep MK-7, MK-4, K1 and vitamin D3 separate, then check whether form and potency testing actually supports the label.

Vitamin K2 dose: separate K2 from total vitamin K guidance

The UK does not publish a K2-specific recommended intake. NHS guidance says adults need approximately 1 mcg/kg/day of total vitamin K, while the UK adult label NRV is 75 mcg total vitamin K. [1] [2] Neither number is an MK-7 or MK-4 target.

Study amounts vary sharply by form. MK-7 trials have used 180, 360 and 375 mcg/day, whereas MK-4 studies include 1.5 mg/day and pharmacological 45 mg/day. [5] [6] [7] [8] Because 45 mg equals 45,000 mcg, collapsing these studies into one K2 range would be misleading.

NHS says 1 mg/day or less of vitamin K supplements is unlikely to cause harm, but also says evidence on high-dose supplementation is limited. [1] Treat that as general vitamin-K safety guidance, not a K2-specific upper limit.

What the Vitamin K2 evidence shows

Vitamin K2 evidence is form- and population-specific. One three-year MK-7 trial at 180 mcg/day reported less decline in some bone measures in healthy postmenopausal women, while another three-year trial at 375 mcg/day added to vitamin D and calcium improved osteocalcin carboxylation but did not improve BMD or bone microarchitecture. [5] [6]

Meta-analyses of postmenopausal osteoporosis trials report a bone signal, but the evidence pools MK-4, MK-7, therapeutic and combination interventions. [9] Cardiovascular evidence has also been mixed, although a 2026 MK-7 trial found slower coronary-calcium progression in symptomatic CAD; whether that changes heart-attack or mortality risk is unknown. [10] [11]

Bone health

Some K2 studies improve vitamin-K-dependent bone markers and some BMD outcomes, but not every trial is positive. [5] [6] MK-4 osteoporosis trials at 45 mg/day belong to a pharmacological treatment context rather than ordinary nutritional supplementation. [8]

Vascular calcification

Earlier controlled-trial reviews found inconsistent effects on calcification and arterial stiffness. [10] A newer 2026 RCT using MK-7 360 mcg/day for two years found slower coronary artery calcium progression in people with symptomatic coronary disease, but clinical-event benefit remains unproven. [11]

General wellness

Total vitamin K is essential for normal clotting, but the available evidence does not justify presenting a K2 supplement as universally necessary for healthy adults who meet overall vitamin K needs.

When to take Vitamin K2

No universal morning or evening timing advantage was identified. Follow the product directions. Vitamin K is fat-soluble, but a label direction to take a capsule with food should be treated as that product's instruction rather than a category-wide efficacy rule.

Warfarin is the main UK safety issue. NHS guidance says not to take vitamin K supplements while taking warfarin and to speak to the treating team before changing medicines or supplements. [3]

NHS says there is not enough evidence to define the effects of high-dose vitamin K supplements and that 1 mg/day or less is unlikely to cause harm. [1] This is general vitamin-K guidance, not a licence to self-use pharmacological MK-4 doses.

Vitamin K2 with D3 and other nutrients

K2 is commonly sold with vitamin D3, but the two doses should remain separate. A current UK combination example provides 90 mcg MK-7 plus 25 mcg D3 among other ingredients. [15]

NHS vitamin D guidance provides its own intake and upper-dose advice and does not establish a universal requirement to add K2 to ordinary vitamin D supplementation. [18] K2 should not be presented as protection against excessive vitamin D.

Who might consider Vitamin K2?

Adults comparing K2 for bone-related or vascular-calcification research questions should look for a product whose exact form resembles the evidence they are reading. A 100 mcg MK-7 capsule, a 100 mcg MK-4 capsule and pharmacological 45 mg MK-4 are not evidence-equivalent.

People using warfarin or other clinician-managed vitamin-K-antagonist therapy should not use a retail K2 comparison as a dosing guide. [3]

How to compare Vitamin K2 testing evidence

Useful K2 testing starts by confirming which form is present and how much is in the finished product. For MK-7, an all-trans claim can be checked with isomer-specific chromatography rather than accepted from marketing alone. Analytical studies have demonstrated measurable differences in MK-7 isomer profiles and label potency. [12] [13]
